MENTAL HEALTH SERVICES

 

If you have concerns for the mental health of the person you care for, you and they should approach their GP, who can refer to a number of different services, both within Primary care, and joint services with 5 Boroughs Partnership, who provide mental health services in St Helens.  St Helens services now follow a 'Road to Recovery' pathway.  The initial access into services is via the Access and Advice team 01744 626661, and referrals can be by GP, Primary Mental Health Team, ASCH, other agency, or by individuals and Carers.  The team is made up of Community Psychiatric Nurses, and are based at Birch House, Peasley Cross.  They will do an initial assessment and then refer on to teams involved in the provision of services, such as the Primary Care Mental Health Team, Community Teams, Early Intervention in Psychosis, Enhanced Day Therapies, Crisis Resolution and Home Treatment, Assertive Outreach, Social Inclusion and the Hope and Recovery Centre.  They will also refer to the voluntary and independent sector

PRIMARY CARE

Services delivered by PCT.  See patients over 16, no upper age limit.  Referral by GP

  • GRADUATE WORKERS - see patients with mild to moderate mental health problems.  Workers visit practices
  • GATEWAY WORKERS - see patients with severe mental illness who are cared for solely in Primary Care.  They do assessments in the practice which can cover issues such as medication management, risk assessment and relapse prevention
  • GP WITH SPECIAL INTEREST - Holds clinics in various Community settings.  Can provide alternative if patients does not wish to receive support from own GP.  Gives GP opportunity of requesting second opinion
  • WELL BEING SUPPORT SERVICE - This new service is for those on their GP's register of patients with Severe Mental Illness.  Specialist Nurses go into GP practices and hold sessions where patients on the register can have a review of their general health, lifestyle assessment, side effect rating of current medication and a discussion on individual needs
  • COUNSELLOR/THERAPIST - Book therapy, guided self help, Cognitive Behaviour Therapy, Group interventions, Depression management, Anxiety management
  • ANXIETY MANAGEMENT - Series of 8 sessions, held in group format at Walk in Centre.  See patients with certain mild to moderate mental health problems

    COMMUNITY MENTAL HEALTH AND HOSPITAL SERVICES FOR ADULTS AND OLDER PEOPLE

    • SOCIAL INCLUSION TEAM - 01744 677034 - Based at Heath Park Lodge, Thatto Heath.  Drop in or referral by Care Coordinator.  Support people 18 and over who have experienced mental health problems and wish to access non mental health services.  The team will Introduce clients to educational and employment opportunities, voluntary, leisure and social activities.  The Social Inclusion workers will help people to exercise choice and to plan, thus developing confidence and self esteem
    • CRISIS RESOLUTION AND HOME TREATMENT TEAM - 01744 677153 9am to 9pm daily, 01925 664000 other times.  .  Based at Lord Street, St Helens.  Referral usually by GP or Care Coordinator.  Team work with those 16 and over who are suffering mental health crisis.  Team provide home based interventions, usually not longer than 4 weeks.  They aim to get the person back to how they felt before the crisis, and prevent hospital admission
    • EARLY INTERVENTION IN PSYCHOSIS TEAM - 01744 677153 Based at Lord Street, St Helens. Open Referrals.  Team work with those 14 to 35 who have or are suspected to have a First Episode of Psychosis, including those with drug/alcohol issues.  Work with clients for up to three years, aiming for social inclusion, prevent relapse and provide family interventions and support as well
    • ASSERTIVE OUTREACH TEAM - 01744 677153 Based at Lord Street, St Helens.  Referrals from Care Coordinators and other teams.  Work with people who have a severe and enduring mental illness, who have found it difficult to engage with other services, who could be at risk of neglect or who could be a risk to themselves or others.  The team aim to assist their clients to live independently in the community whilst receiving the support and services they require
    • ENHANCED DAY THERAPIES - 01744 458457 Based at Birch Day Unit.  Referrals from Access and Advice Team or Care Coordinator.  Work with people with severe and enduring mental illness, or a serious mental health issue such as Post Traumatic Stress disorder or complex trauma.  Have Psychologists, Cognitive Behavioural Therapists and Occupational Therapists.  Look at daily living skills, social network roles and employment skills

  • Services provided at The Elms, Cowley Hill Lane 01744 454368. Referral is via GP, Health Visitor, School or other health professional

  • COMMUNITY LINK - 612411 - www.c-i-c.co.uk - provide advice, support counselling and treatment for addictions, including alcohol, gambling and drugs.  Based at Wolverhampton House, Church Street.  Counselling, advice and support groups for families of addicted person
  • CONNECTED CARERS - 454802 - Project which supports Carers aged 8 - 18 who care for someone with a mental health problem.  Provides opportunities to meet other young Carers, make friends and have some time away from caring.
  • DUAL DIAGNOSIS DROP IN CENTRES - 01925 291094, Chris Ollerton and 222624, Brian Roscoe - Meet 1st and 3rd Tuesday 12:30pm at Newton Library,  2nd Thursday 1:00pm at the Gamble building (Within Central Library), and 1st Monday 1:00pm at Moss Bank Library.  You can accompany the person you care for when they attend the meetings.  Meetings aim to build the person's confidence and explore service help available 
  • FAMILY WELFARE ASSOCIATION - www.fwa.org.uk - A family charity, supporting families to cope with complex issues including domestic abuse, parents with mental illness, learning disabled parents and severe financial hardship

  • MIND - Mental Health Charity.  601204 - Offices in College Street, and a base at Heath Park Lodge, Thatto Heath.  Support people with experiences of mental distress to overcome barriers, access training, volunteering and employment.  Run a Befriending Scheme which puts those with mental distress issues in contact with volunteer befrienders who provide social support and encourage confidence and social networking. All calls answered by trained volunteers (approved by Royal College of Psychiatrists) Support and information about mental illness, symptoms and treatments and services available in local area, support and reassurance during a crisis, for both sufferers and their Carers and families.   • ST HELENS MENTAL HEALTH CARERS - 758678 - www.sthelensmhcarers.org Based at United Reform Church. Carers Reference Group meet on the second Wednesday of the month, commencing at 6pm.  For Carers who wish to contribute to policy making and have a more active role in Mental Health Carer issues.  A member of the Carer Support Team will accompany you on your first visit if you wish, please contact the centre, 01744 675615
  • TOGETHER - 734883 - TOGETHER is national charity providing a range of mental health services.  In St Helens they have been commissioned to provide a Carer Support Service to support Carers, a Respite Care Service to offer regular breaks to Carers, and Community Support Services to provide support for people with mental health needs.  These services involve visiting you at home and agreeing a plan of support, which can last 6-8weeks for respite and around three months for Carer Support, and two years for Community Support.  Anyone can approach the service and request support   www.together-uk.org.uk
