CARE ASSESSMENTS
Social Care teams provide
Social Work support for older people, provide and purchase
residential care for older people, provide day care for
older people and a wide range of community based support
services to older people in order to promote their
independence and support them in the community. Services are
co-ordinated by three community teams, a hospital based team
and the Domiciliary care commissioning unit.
If you wish to request
a referral for these services you can do so via the Contact Centre
Advisors (Social Services) who are on duty from 8.45 a.m. until 5
p.m. Monday to Friday. they can be contacted by telephone on (01744)

or personal visit to the Contact Centre, Wesley house,
Corporation Street, St. Helens WA10 1HF
- Domiciliary care
commissioning unit - 677712 - if the person you care for is
in receipt of a care package, and you feel that circumstances
have changed, contact the Unit. Their primary function
is to complete individual service user reviews, mainly for
people over the age of 65, were there is no involvement from
a care manager or social worker, to ensure that care
packages are appropriate to meet individuals assessed need

CENTRES
Day services are provided at
Kershaw Centre -
leaflet available, for older people with
physical or mental frailty - Contact Centre referral
required to access a Community Care Assessment, which will
identify if Day Care needed
THE
REABLEMENT TEAM
Based at Alexander Court, Prescot Rd,
Tel 620378. The aim of the service is to help
those 55+ to remain as independent as possible. They
help people leaving hospital, following illness at home,
bereavement, loss of confidence with daily living skills and
after falls. They work with clients to prevent hospital or
care home admissions by promoting mobility, confidence, diet
and personal care skills. The team can work with client for 6-8
weeks in their home. Also run a rapid response service
for 18+ age group, respond to referrals within 2 hours.
This may be suitable if the person you care for experiences sudden illness and then
needs support to stay at home, and may also be suitable if main Carer
suddenly became unable to care e.g. rushed to hospital with
heart attack. Support available for 7 days on this
scheme. Referrals can come from GP, Nurse, Social Care,
Hospital and Voluntary Agencies for both schemes

and Crafts, Holistic Therapy sessions, Beauty Therapy
appointments and exercise and dance sessions

services and equality
BEFRIENDING SCHEME
- 752644 - Staff visit
elderly, housebound people, assess them and then volunteers
visit on a regular basis. Those referred should be
able to understand the scheme

CARE AND REPAIR - Promotes independence by providing advice
and help to home owners and private tenants who are older,
disabled or vulnerable. Helps with repairs, improvements
and adaptations. Advice is free, charge for estimates and
for work done, but Arena will advise on benefits and grants
available HANDYPERSONS SCHEME
- Assistance for older, disabled and low income families to
carry out minor repair work. Also carry out accident
prevention work. Charge for materials only. Will
refer to Care and Repair if not suitable for Handyperson scheme PAINTING
AND GARDENING SCHEME - Clients have to be aged 65+ and on means tested benefit

their families. It is designed to ensure that care
residents and their families have access to all the independent
help and information they will need in dealing with long term
care needs

DAWN PATROL

card in window each morning and volunteer school children
are asked to check. If no card visible, they inform
responsible person at school, and health or social care
staff are alerted

FALLS PREVENTION
- 01744 457231/607432 - Offer various services to
assess and support elderly people and their carers.
These range from a programme of supervised exercise to improve
balance and co-ordination to assessment of footwear, medication
and diet. A referral from a GP, District Nurse or other
health professional is required

FUND FOR THE AGED - 020 7828 0200 - Direct, practical
assistance for elderly on low incomes through the provision of
free holidays, emergency alarms and pain relief machines.

- Response service which provides telephone and pendant
alarms, which link to a switchboard that has details of
emergency contacts given by the user of the service. These
people have to live close by. Costs incurred for
purchase of unit (£ 184.48) are not incurred if savings are
under £ 20,000, the person is over 60, and does not live in
residential sheltered accommodation. Service charges
are paid by everyone and cost £ 1.63+vat/week

then £ 3 per hire session. Need two forms
of ID to hire equipment. Also do long term loan
