GP Work

 

Most Carers tell us that their Family Doctor is often their first port of call when they are looking for information. The Centre has a Carers GP Link Officer which is part of the Halton & St Helens National Carer Demonstrator Site to improve NHS Services to Carers.

The aim of the work is to ensure GPs:-
· Identify those patients who are Carers.
· Recognise the role that Carers played in supporting the cared for person.
· Provide information and advice to support them in their caring role.


IDENTIFICATION
We are currently working with GP practices in St Helens to assist them in identifying Carers. Practices are provided with posters and leaflets to encourage patients to self identify
 

INFORMATION
Carers identified that they need to be directed to good sources of information, help and support. All practices are supplied with St Helens Carers Centre leaflets and have also been offered Carer Information Boards which are funded by the Carers Centre.
 

REFERRAL
Practices are asked to refer Carers directly to the Carers Centre. We recognise that each practice works differently and staff are encouraged to use whatever way works best for them to refer their patients. This enables us to interview the Carer directly and provide specific information tailored to their individual needs.
 

CARER AWARENESS TRAINING
All GP practices are offered Carer Awareness Training. The aim of the training is to help staff to identify who is a Carer, raise awareness of the role and health needs of Carers and provide information about the services provided by St Helens Carers Centre.
