COMMUNITY CARE ASSESSMENTS Social Care teams provide Social Work support for older people, provide and purchase residential care for older people, provide day care for older people and a wide range of community based support services to older people in order to promote their independence and support them in the community. Services are co-ordinated by an Access and Information team, three community teams, a hospital based team and the Review team. If you wish to request a referral for these services you can do so via the Contact Centre Advisors (Social Services) who are on duty from 8.45 a.m. until 5 p.m. Monday to Friday. they can be contacted by telephone on (01744) 456600, e-mail to contactcentre@sthelens.gov.uk| or personal visit to the Contact Centre, Wesley house, Corporation Street, St. Helens WA10 1HF
DAY CENTRES Day services are provided at Kershaw Centre in Newton Le Willows, for older people with physical or mental frailty. A Contact Centre referral is required to access a Community Care Assessment, which will identify if Day Care is needed. Other Centres used are Moss Bank and Brookfield THE REABLEMENT TEAM The aim of the service is to help those 55+ to remain as independent as possible. They help people leaving hospital, following illness at home, bereavement, loss of confidence with daily living skills and after falls. They work with clients to prevent hospital or care home admissions by promoting mobility, confidence, diet and personal care skills. The team can work with client for 6-8 weeks in their home. Offer telephone advice, appointments at office, and home visits if necessary ACTIVE CARE - 752644 (Mansion House and various community venues) Provide physical and mental stimulation activities in safe, friendly environments. Classes, lunch and snacks are included in the price of attendance ACTIVE AGE - 752644 (Mansion House and various community venues) Improve physical and mental health and reduce social isolation by attending activities such as IT skills, Arts and Crafts, Holistic Therapy sessions, Beauty Therapy appointments and exercise and dance sessions SENIOR VOICE FORUM - 752644 - a voice for people 50+, campaigning for better services and equality BEFRIENDING SCHEME - 752644 - Staff visit elderly, housebound people, assess them and then volunteers visit or telephone on a weekly basis. May accompany the person to shops or trip out, or just visit in the home. Those referred should be able to understand the scheme OTHERS ARENA OPTIONS (NEW NAME FOR ARENA SERVICES) Tel: 0845 142 2064 CARE AND REPAIR - Promotes independence by providing advice and help to home owners and private tenants who are older, disabled or vulnerable. Helps with repairs, improvements and adaptations. Advice is free, charge for estimates and for work done, but Arena will advise on benefits and grants available HANDYPERSONS SCHEME - Assistance for older, disabled and low income families to carry out minor repair work. Also carry out accident prevention work. Charge for materials only. Will refer to Care and Repair if not suitable for Handyperson scheme PAINTING AND GARDENING SCHEME - Clients have to be aged 65+ and on means tested benefit CARE AWARE - 08705 134925 - Provide an advocacy service for Care residents and their families. It is designed to ensure that care residents and their families have access to all the independent help and information they will need in dealing with long term care needs www.careaware.co.uk CARE DIRECTIONS - www.caredirections.co.uk - This site aims to offer information and support to older people and their families as they make choices about where to live, how to obtain support, health service issues and financial issues COMMUNITY ACTIVITY INFORMATION - link to document - List of community groups across the borough, social groups, craft activity groups etc. COUNSEL AND CARE - www.counselandcare.org.uk charity that provides advice and support to older people by telephone, email or letter regarding finances, housing and care homes. Link on website to useful factsheets DAWN PATROL - 01925 292740 - Partnership scheme for older residents. Place a card in window each morning and volunteer school children are asked to check. If no card visible, they inform responsible person at school, and health or social care staff are alerted ELDER ABUSE - Confidential helpline run by trained staff and volunteers - 0800 808 8141 FALLS PREVENTION – 01744 457238 - A service provided by the Primary Care Trust. Anyone concerned about falls or who has a history of falls can refer into the programme. All clients given falls prevention information around diet, exercise and footwear FIRST STOP - 0800 377 7070 - Advice service for older people and their families. Scheme to ensure that correct information on illnesses and medication accompanies Cared For in an emergency NATIONAL BENEVOLENT FUND FOR THE AGED - 020 7828 0200 - Direct, practical assistance for elderly on low incomes through the provision of free holidays, emergency alarms and pain relief machines. Can download application forms for holidays from website www.nbfa.org.uk SENIOR LINK - 0845 053 2306 - www.helptheaged.org.uk/seniourlink - Response service which provides telephone and pendant alarms, which link to a switchboard that has details of emergency contacts given by the user of the service. These people have to live close by. Costs incurred for purchase of unit (£ 184.48) are not incurred if savings are under £ 20,000, the person is over 60, and does not live in residential sheltered accommodation. Service charges are paid by everyone and cost £ 1.63+vat/week SHOPMOBILITY - 613388 - open Mon to Sat. £ 10 to register membership, then £ 3 per hire session. Need two forms of ID to hire equipment.
