ST HELENS INTEGRATED CANCER CARE PROGRAMME - 611501 - This service aims to improve the quality of care for those who have cancer.  The service coordinator acts as a point of contact for community services such as District Nurses and Cancer Nurse specialists.  She will arrange for an assessment at home to agree a programme of visits by the District Nurses, and the programme could also include help with appointments, treatment and medication.  Referrals can be made by patients and families as well as health care staff

MACMILLAN In Partnership with Age UK & Age Concern (Liverpool & Sefton) - Specialist service to support over 60's with cancer - information, advice & signposting, plus practical support - light housework, shopping, hospital discharge support. Also help to identify welfare benefit entitlement & other financial support. Referral by Specialist Nurse or Keyworker. Or contact Age Concern Liverpool & Sefton on 0151 256 2556.
